By default, ansible gathers facts from facter and ohai if installed on
the remote nodes, given we don't need them, let's exclude these facts
from our facts gathering

There's no need to have two plays anymore since we now set/unset osd
flags in `ceph-osd` role.
Also, this commit makes the role `ceph-facts` to be called after
`ceph-defaults`

This commit makes this playbook gathering facts from all other nodes but
clients.
When collocating OSDs on other nodes it can fail like following:
```
fatal: [vm252-11]: FAILED! => {
"msg": "'ansible.vars.hostvars.HostVarsVars object' has no attribute 'ansible_hostname'"
}
```
In that case, a fact from a RGW node is called when rendering the
`ceph.conf.j2` but it fails because facts are gathered only from mon and
osd nodes.

ceph-facts should be run before we play ceph-validate since it has
reference to facts that are set in ceph-facts role.

otherwise, it will end up with error like following:

sometimes we play the whole role `ceph-defaults` just to access the
default value of some variables. It means we play the `facts.yml` part
in this role while it's not desired. Splitting this role will speedup
the playbook.

Three fixes:
- fix a typo in vagrant_variables that cause a networking issue for
containerized scenario.
- add containerized_deployment: true
- remove a useless block of code: the fact docker_exec_cmd is set in
ceph-defaults which is played right after.

The playbook has various improvements:
* run ceph-validate role before doing anything
* run ceph-fetch-keys only on the first monitor of the inventory list
* set noup flag so PGs get distributed once all the new OSDs have been
added to the cluster and unset it when they are up and running
