* using separated vault roles for generate certs with different `O` (Organization) subject field;
* configure vault roles for issuing certificates with different `CN` (Common name) subject field;
* set `CN` and `O` to `kubernetes` and `etcd` certificates;
* vault/defaults vars definition was simplified;
